    SMEs in Focus : Exploring the experience of micro, small and medium enterprise owners in Malta

    Exploring help seeking behaviour in health : are gender binaries useful?

    While positivistic research uses male/female, sex or gender as the independent variable to highlight its associations with health experience, qualitative research shows that binary notions of male/ female sex and gender are sometimes insufficient markers of lived reality. This article aims to highlight gendered aspects of the consultative relations that have been shown to have an impact on subsequent medical care provision. It outlines the dynamics within consumer/medical professional relations that are impacted by gender stereotype associations using cardiac symptoms as an example, and then goes on to problematize the binary concept of gender and its impact on health care provision.peer-reviewe

    Banking on you? : the level of public awareness of biobanks in Malta

    This paper explores the level of awareness of the Maltese general public in relation to the existence and use of biobanks as resources for biomedical and genomic research. Using a quantitative research design, a four question survey was administered face-to-face to a random stratified quota sample (n = 387) of the Maltese population. The survey assessed whether the general public understands what a biobank is, and what the people believe/ think a biobank might be. Results show that the overwhelming majority of the public is not aware of the term `biobank', and when asked to think about what a biobank could be, the majority of these failed to give an accurate answer, with a financial institution being the most frequent suggestion. This said, 26.5% of those who initially claimed that they did not know what a biobank is (or claimed they were not sure) went on to give a legitimate response when asked to speculate about what a biobank could be. Most of these respondents mentioned biobanks which store gametes and/or embryos and biobanks which store blood and organs for the purpose of donation. Whilst gender does not seem to be a significant factor in the outcome as an independant variable, educational achievement did have an influence on the accuracy of the responses. The key finding is that only 2.3% of the Maltese population promptly associated the term `biobank' with a facility for storing tissue for the purpose of biomedical research, while the vast majority believed it referred to a financial institution.peer-reviewe

    Dwarna : a blockchain solution for dynamic consent in biobanking

    Dynamic consent aims to empower research partners and facilitate active participation in the research process. Used within the context of biobanking, it gives individuals access to information and control to determine how and where their biospecimens and data should be used. We present Dwarna—a web portal for ‘dynamic consent’ that acts as a hub connecting the different stakeholders of the Malta Biobank: biobank managers, researchers, research partners, and the general public. The portal stores research partners’ consent in a blockchain to create an immutable audit trail of research partners’ consent changes. Dwarna’s structure also presents a solution to the European Union’s General Data Protection Regulation’s right to erasure—a right that is seemingly incompatible with the blockchain model. Dwarna’s transparent structure increases trustworthiness in the biobanking process by giving research partners more control over which research studies they participate in, by facilitating the withdrawal of consent and by making it possible to request that the biospecimen and associated data are destroyed.peer-reviewe

    Moving back to the future of big data-driven research : reflecting on the social in genomics

    With the advance of genomics, specific individual conditions have received increased attention in the generation of scientific knowledge. This spans the extremes of the aim of curing genetic diseases and identifying the biological basis of social behaviour. In this development, the ways knowledge is produced have gained significant relevance, as the data-intensive search for biology/sociality associations has repercussions on doing social research and on theory. This article argues that an in-depth discussion and critical reflection on the social configurations that are inscribed in, and reproduced by genomic data-intensive research is urgently needed. This is illustrated by debating a recent case: a large-scale genome-wide association study (GWAS) on sexual orientation that suggested partial genetic basis for same-sex sexual behaviour (Ganna et al. 2019b). This case is analysed from three angles: (1) the demonstration of how, in the process of genomics research, societal relations, understandings and categorizations are used and inscribed into social phenomena and outcomes; (2) the exploration of the ways that the (big) data-driven research is constituted by increasingly moving away from theory and methodological generation of theoretical concepts that foster the understanding of societal contexts and relations (Kitchin 2014a). Big Data Soc and (3) the demonstration of how the assumption of ‘free from theory’ in this case does not mean free of choices made, which are themselves restricted by data that are available. In questioning how key sociological categories are incorporated in a wider scientific debate on genetic conditions and knowledge production, the article shows how underlying classification and categorizations, which are inherently social in their production, can have wide ranging implications. The conclusion cautions against the marginalization of social science in the wake of developments in data-driven research that neglect social theory, established methodology and the contextual relevance of the social environment.peer-reviewe

    Collaboration and teamwork: immersion and presence in an online learning environment

    In the world of OTIS, an online Internet School for occupational therapists, students from four European countries were encouraged to work collaboratively through problem-based learning by interacting with each other in a virtual semi-immersive environment. This paper describes, often in their own words, the experience of European occupational therapy students working together across national and cultural boundaries. Collaboration and teamwork were facilitated exclusively through an online environment, since the students never met each other physically during the OTIS pilot course. The aim of the paper is to explore the observations that here was little interaction between students from different tutorial groups and virtual teamwork developed in each of the cross-cultural tutorial groups. Synchronous data from the students was captured during tutorial sessions and peer-booked meetings and analysed using the qualitative constructs of ‘immersion’, ‘presence’ and ‘reflection in learning’. The findings indicate that ‘immersion’ was experienced only to a certain extent. However, both ‘presence’ and shared presence were found by the students, within their tutorial groups, to help collaboration and teamwork. Other evidence suggests that communities of interest were established. Further study is proposed to support group work in an online learning environment. It is possible to conclude that collaborative systems can be designed, which encourage students to build trust and teamwork in a cross cultural online learning environment.</p

    1D-3D hybrid modeling—from multi-compartment models to full resolution models in space and time

    Investigation of cellular and network dynamics in the brain by means of modeling & simulation has evolved into a highly interdisciplinary field, that uses sophisticated modeling & simulation approaches to understand distinct areas of brain function. Depending on the underlying complexity, these models vary in level of detail to cope with the attached computational cost. Hence for large network simulations, single neurons are typically reduced to time-dependent signal processors, dismissing spatial aspects of the cells. For single cell or small-world networks, general purpose simulators allow for space and time-dependent simulations of electrical signal processing, based on the cable equation theory. An emerging field in Computational Neuroscience encompasses a new level of detail by incorporating the 3D morphology of cells and organelles into 3D space and time-dependent simulations. Every approach has its advantages and limitations, such as computational cost, integrated and methods-spanning simulation approaches, depending on the network size could establish new ways to investigate the brain. We present a hybrid simulation approach, that makes use of reduced 1D-models using e.g. the NEURON which couples to fully resolved models for simulating cellular and sub-cellular dynamics, including the detailed 3D-morphology of neurons and organelles. To couple 1D- & 3D-simulations, we present a geometry and membrane potential mapping framework, with which graph-based morphologies, e.g. in swc-/hoc-format, are mapped to full surface and volume representations of the neuron; membrane potential data from 1D-simulations are used as boundary conditions for full 3D simulations. Thus, established models and data, based on general purpose 1D-simulators, can be directly coupled to the emerging field of fully resolved highly detailed 3D-modeling approaches.     Diabetes care provision in UK primary care practices

    Background: Although most people with Type 2 diabetes receive their diabetes care in primary care, only a limited amount is known about the quality of diabetes care in this setting. We investigated the provision and receipt of diabetes care delivered in UK primary care. Methods: Postal surveys with all healthcare professionals and a random sample of 100 patients with Type 2 diabetes from 99 UK primary care practices. Results: 326/361 (90.3%) doctors, 163/186 (87.6%) nurses and 3591 patients (41.8%) returned a questionnaire. Clinicians reported giving advice about lifestyle behaviours (e.g. 88% would routinely advise about calorie restriction; 99.6% about increasing exercise) more often than patients reported having received it (43% and 42%) and correlations between clinician and patient report were low. Patients’ reported levels of confidence about managing their diabetes were moderately high; a median (range) of 21% (3% to 39%) of patients reporting being not confident about various areas of diabetes selfmanagement. Conclusions: Primary care practices have organisational structures in place and are, as judged by routine quality indicators, delivering high quality care. There remain evidence-practice gaps in the care provided and in the self confidence that patients have for key aspects of self management and further research is needed to address these issues. Future research should use robust designs and appropriately designed studies to investigate how best to improve this situation
